Our office is proud to offer electronically submitted dental claims. This allows you, the patient, the ease and convenience of receiving your reimbursement from your insurer in a rapid effective manner. To do so, we need the following information, generally available to the plan subscriber in the form of a benefits card, which you may bring along to your initial visit:

1. Name of the dental insurance company
2. Policy or group number
3. Certificate or subscriber ID number

Your dental insurance is a contract between you and the insurance company. Our goal is to provide you with the treatment you need based on our findings. Your insurance is in place to help you cover some of the costs of the treatment. We have included a list of pertinent questions that all patients should ask their insurer about their coverage. These will help keep you informed of the details of your plan.

1. What is the annual maximum per patient?
2. What is the anniversary date of the policy? Example: January 1st
3. Is there an annual deductible? If yes, how much is it?
4. Dental benefits are paid on which year's Ontario Dental Association fee schedule?
5. How many units of scaling and/or root planing are covered per year? Which codes are allowed for these services?
6. How many recall appointments are allowed annually? Example every 3 months, 6 months or 9 months?
7. What percentage of coverage is allowed for the following:
   • Diagnostic services
   • Preventative services
   • Restorative services
   • Endodontic services
   • Periodontal services
   • Major treatment Example crowns, bridges, dentures
   • Treatment Planning/Consultations
8. What is the annual maximum for major treatment?
9. Is endodontic and/or periodontal treatment classified as basic or major treatment
